Job summary

FOX Rehabilitation is expanding in Concord, MA, seeking licensed Physical Therapists to deliver one-on-one care to older adults in private residences or senior living communities. Join a clinician-operated practice, with autonomy to create your own schedule and a focus on meaningful outcomes.

Responsibilities include evaluations, plan-of-care development, and delivering evidence-based therapy, with potential telehealth integration for flexible patient access.

Qualifications

• Licensed PT in MA or eligibility to obtain
• Degree from an accredited Physical Therapy program
• EMR proficiency and basic computer skills
• CPR certification maintained

Responsibilities

• Provide one-on-one physical therapy to older adults in home or community settings
• Perform evaluations and develop individualized plans of care
• Deliver therapy sessions focused on mobility and independence
• Document in FOX EMR per regulatory standards
• Collaborate with PTs, OTs, SLPs, and Fitness Specialists
• Educate patients, caregivers, and staff to promote safety and aging in place
• Provide telehealth/virtual care when appropriate
