Job summary

Therapy Management Corporation (TMC) is seeking a licensed Physical Therapist to empower adult and geriatric patients through compassionate, patient-centered care within a collaborative clinical environment.

You will provide physical therapy services with clinical autonomy, focusing on improving independence, mobility, safety, and quality of life for older adults in a supportive team setting.

Qualifications

• Licensed Physical Therapist (PT) or license eligible in the state of practice.
• Graduate of an accredited Physical Therapist Program.
• Interest or experience in skilled nursing, long‑term care or geriatric rehabilitation.
• Passion for patient-centered care and improving quality of life for older adults.

Responsibilities

• Evaluate patients and provide personalized physical therapy treatments to improve mobility and independence.
• Develop and implement patient-centered treatment plans focused on long-term rehabilitation goals.
• Deliver hands-on therapy interventions to support recovery and functional quality of life in older adults.
• Help patients restore functional mobility and safely perform daily activities with confidence.
• Partner with interdisciplinary teams, caregivers, and nursing staff to drive meaningful outcomes and continuity of care.
• Track patient progress, maintain accurate clinical documentation, and adapt plans as needed.
