Qualifications

  • Candidate must be licensed as a Physical Therapist (or eligible for licensure).
  • Graduate of an accredited Physical Therapist program.
  • Interest or experience in skilled nursing, long-term care or geriatric rehabilitation.
  • Passion for patient-centered care and improving quality of life for older adults.

Responsibilities

  • Evaluate patients and provide personalized physical therapy treatments to improve mobility, strength, balance, and independence.
  • Develop and implement patient-centered treatment plans focused on long-term rehabilitation goals.
  • Deliver hands-on therapy interventions to support recovery and functional quality of life in older adults.
  • Help patients restore functional mobility and safely perform daily activities with greater confidence.
  • Collaborate with interdisciplinary teams, caregivers, and nursing staff to drive outcomes and continuity of care.
  • Track patient progress and maintain accurate clinical documentation to adapt plans as needed.
