Job summary

Ignite Rehab, operating as Metro Physical & Aquatic Therapy, is seeking a motivated per diem Occupational Therapist to provide in-home care in Morris County, NJ. All levels of experience are welcome; mentorship is offered to support documentation and compliance.

Qualifications

• OT Program graduate required.
• Licensed OT in NJ or eligible.
• Home health experience preferred; mentor provided for documentation and compliance.
• Ability to travel to patients' homes within the assigned geographic territory.
• Strong communication with patients, support staff, and management, including timely responses via text, email, phone, etc.

Responsibilities

• Accept and treat patients within your designated geographic area.
• Provide 1:1 treatment.
• Evaluate patients and develop individualized treatment plans based on physician orders and patient needs.
• Deliver care that aligns with professional standards, agency policies, and the prescribed plan of care.
• Educate family members, home health aides, and other caregivers on proper therapy techniques.
• Plan, and carry out occupational therapy programs to maintain, improve or restore physical function, relieve pain, and prevent occupational dysfunction in patients.
• Perform and document an initial exam, evaluating the data to identify problems and determine a diagnosis prior to intervention.
• Provide timely and accurate documentation for each visit.
• Evaluate effects of treatment at various stages and adjust treatments to achieve maximum benefit.
• Must be willing to treat geriatric patients.
