Job summary

BrightStar Care – Concord, Lexington & Woburn is seeking a licensed Physical Therapist to join our home health team for part-time or per diem work. You will evaluate clients in their homes, develop personalized plans, and provide skilled interventions to improve strength, balance, mobility and safety.

The role requires an MA PT license, CPR certification, and reliable transportation. Experience in home health and a passion for patient education are valued traits.

Qualifications

• Active MA PT license.
• DPT or MPT from an accredited program.
• CPR certification.
• 1+ year of experience preferred (new grads welcome).
• Reliable vehicle and valid driver’s license.

Responsibilities

• Perform in-home PT evaluations and create personalized care plans.
• Provide skilled PT interventions to enhance strength, balance, mobility, and safety.
• Educate clients and families on exercises and home safety.
• Collaborate with a multidisciplinary care team.
• Document care using a mobile-friendly EMR.
