Job summary

Signify Health, part of CVS Health, is seeking a Full Time Physician to provide in-home health evaluations, engaging with patients in the comfort of their own homes and bridging gaps in care.

The visit lasts about 45 minutes and includes a medication and medical history review, a physical evaluation, and, if ordered, additional diagnostic tests. You’ll use an iPad with a simple clinical workflow; no prescriptions or lab orders are required.

Qualifications

• Active, unrestricted medical license in applicable states.
• Multi-state licenses preferred.
• In-home clinician role requires patient-centered care.

Responsibilities

• Visit patients in their homes to conduct health evaluations.
• Perform medication and medical history reviews during visits.
• Coordinate with health plans to connect patients with needed care.
• Travel locally within licensed states; conduct virtual visits when needed.
• Utilize iPad for clinical workflow; no prescriptions or tests required.
