Job summary

HarmonyCares seeks a qualified Nurse Practitioner to deliver in-home primary care to patients with complex needs. You will work in a team-based, physician-driven model and focus on thorough, compassionate care in patients’ homes.

The role requires NP licensure in the relevant state, DEA eligibility, and CPR/BLS certification. Geriatric experience and a valid driver’s license are highly valued.

Qualifications

• Licensed as an NP in the state where HarmonyCares Medical Group operates.
• Federal DEA license/eligibility.
• Geriatric training/experience preferred.
• Active CPR/BLS certification.
• Board certification as NP.
• Must maintain a valid driver’s license and good driving record.

Responsibilities

• Provide primary care to high-acuity patients in home settings.
• Practice medicine with a patient-centered focus over volume.
• Take time to address each patient’s individual needs.
