HarmonyCares is seeking a Nurse Practitioner or Physician Assistant to provide comprehensive in-home primary care to medically complex adults in Newark, NJ and surrounding areas. This role emphasizes patient care over volume, with 6–10 patient visits daily and 30–60 minutes per visit to support chronic condition management and relationship-building with patients and families.

Qualifications

• Active NJ NP/PA license or eligibility for licensure.
• Board certification in corresponding field.
• Federal DEA license or eligibility.
• CPR/BLS certification required.

Responsabilités

• Provide comprehensive primary care to medically complex adult patients in their homes.
• Assess, diagnose, and treat acute and chronic conditions.
• Manage patients with multiple comorbidities.
• Order and interpret diagnostic testing.
• Prescribe and manage medications.
• Educate patients and caregivers; collaborate with the interdisciplinary care team.
