Job summary

HarmonyCares in Passaic County, NJ is seeking a Nurse Practitioner to serve patients in their homes, delivering comprehensive primary care to medically complex adults in a value-based, in-home setting.

The role emphasizes quality care over volume, with 6–10 patients per day and 30–60 minute visits, supported by an interdisciplinary team and ongoing professional credentials maintenance.

Qualifications

• Board certification
• Federal DEA license or eligibility
• Active CPR/BLS certification
• Valid driver’s license and good driving record
• Primary care experience with adult patients
• Ability to travel locally throughout the assigned territory
• Experience in geriatrics/home-based care is preferred

Responsibilities

• Assessing, diagnosing, and treating acute and chronic conditions
• Managing patients with multiple comorbidities
• Ordering and interpreting diagnostic testing
• Prescribing and managing medications
• Educating patients and caregivers
• Collaborating with the interdisciplinary care team
