Nurse Practitioner – Home-Based Primary Care | Passaic County, NJ

HarmonyCares is a leading national provider of value-based, in-home primary care for patients with complex healthcare needs. We are seeking a Nurse Practitioner to serve patients in Passaic County, NJ and the surrounding area.

This role is ideal for an NP who wants to focus on patient care, not volume. Our providers typically see 6–10 patients per day, with approximately 30–60 minutes per visit, allowing time to manage complex and chronic conditions and build meaningful relationships with patients and families.
