Job summary

HarmonyCares is seeking a dedicated Nurse Practitioner to deliver high-quality in-home primary care in New Jersey. You will focus on geriatric and complex patients, taking time to address their needs, not just volume, with an emphasis on individualized care plans.

Qualifications

• Licensed as a NP in the State.
• Federal DEA License/ eligibility.
• Geriatric training/experience preferred.
• Active CPR/BLS Certification.
• Board Certification.
• Must maintain a valid driver’s license and good driving record.

Responsibilities

• Provide primary care to high acuity patients in home visits, Mon–Fri.
• Address patient needs with time and attention to quality of care.
• Support home health, geriatrics, hospice experience as applicable.
