Qualifications

Graduate of an accredited NP or PA program.
Current New York State license and National Board Certification.
Active DEA license.
Minimum 3 years of clinical experience in home-based primary care, geriatrics, or complex chronic care.
Experience with value-based care models (ACO, risk-bearing entities).
Familiarity with care coordination and quality reporting in ACO programs.
EMR/EHR proficiency.
Bilingual Spanish strongly preferred.
Ability to work independently in the home setting while collaborating with a team.

Responsibilities

Provide comprehensive primary care to homebound and high-needs patients.
Perform exams, document histories, and develop evidence-based care plans.
Diagnose and treat a wide range of conditions with emphasis on chronic disease management.
Record and maintain data aligned with ACO documentation and quality reporting.
Order and interpret diagnostic tests, labs, and imaging.
Prescribe treatments and administer vaccinations.
Counsel patients on prevention, nutrition, and adherence to plans.
Collaborate with interdisciplinary team members to support coordinated care.
Work with hospital-at-home partners to prevent unnecessary admissions.
Participate in case conferences and patient panel reviews.
Provide timely charting and coding that meets program standards.
Share after-hours emergency hotline coverage on a rotation.
Train under PCPs and specialists to gain experience.
Coordinate with Health Home for Social Determinants of Health support.
