Job summary

Essen Health Care in New York seeks a Nurse Practitioner to deliver advanced primary and specialty care across our growing network. You will help patients in diverse settings, from clinics to home-based care, and contribute to population health initiatives.

Qualifications

Master's degree in a Nurse Practitioner program (Family or Adult Gerontology).
Current New York State NP license.
National board certification required.
AANP and ANCC certifications preferred.
ONCC certification considered.
Spanish speaking skills a plus.
EMR/EHR experience a plus.
New grads encouraged to apply; driving access strongly preferred.

Responsibilities

Examine, diagnose and treat patients across primary and specialty care.
Develop and manage patient care plans and documentation.
Perform physical exams and record medical history.
Order and interpret diagnostic tests and imaging.
Administer treatments and vaccinations as needed.
Collaborate with subspecialists within the network.
Provide care to residents in nursing home settings.
Maintain accurate patient records and CPT coding awareness.
