Qualifications

• Master's degree in a Nurse Practitioner program (Family or Adult Gerontology).
• Current New York State NP license.
• National Board Certification required (AANP/ANCC/ONCC).
• Bilingual Spanish is a plus.
• EMR/EHR experience is a plus.
• New grads are encouraged to apply.

Responsibilities

• Patient evaluations during home visits, including health status, history, and medication reconciliation.
• Diagnose conditions, develop and implement treatment plans, and prescribe medications as needed.
• Coordinate care with physicians, specialists, and other health providers to ensure continuity of care.
• Educate patients and families on health management, lifestyle changes, disease prevention, and medication adherence.
• Maintain accurate documentation of all patient interactions in electronic health records.
• Schedule and conduct follow-up visits to monitor progress and adjust treatment plans.
