Responsibilities

• Perform home-based patient assessments and examinations.
• Diagnose conditions and develop treatment plans for home visits.
• Prescribe medications and therapies as appropriate.
• Coordinate care with physicians, nurses, and the care team.
• Educate patients and families on disease management and prevention.
• Maintain accurate electronic health records for encounters and follow-ups.
• Schedule follow-up visits and adjust treatment plans as needed.
• Assess urgent needs during home visits and coordinate referrals.
• Engage in community outreach to promote home healthcare services.
