HarmonyCares in Jersey City, NJ seeks a Nurse Practitioner or Physician Assistant to provide comprehensive home-based primary care to medically complex adults in Jersey City and surrounding areas.

The role focuses on patient care over volume, with 6–10 visits per day, 30–60 minutes per visit, and a collaborative team. Eligible candidates hold NJ NP or PA license, board certification, DEA, CPR/BLS, and able to travel locally.

Formación

• Active New Jersey NP or PA license.
• Board certification.
• Federal DEA license or eligibility.
• Active CPR/BLS certification.
• Valid driver’s license and good driving record.
• Primary care experience with adults.
• Ability to travel locally throughout the territory.

Responsabilidades

• Provide comprehensive primary care to medically complex adults in patients' homes.
• Assess, diagnose, and treat acute and chronic conditions.
• Develop individualized care plans.
• Order and interpret diagnostic tests.
• Prescribe and manage medications.
• Educate patients and caregivers.
• Collaborate with interdisciplinary care team.
