Job summary

HarmonyCares seeks a Nurse Practitioner to deliver in-home primary care for high-acuity patients in New Jersey. This role emphasizes patient-centered care, not volume, with a focus on geriatrics and home health.

The NP must hold an active NP license with DEA eligibility and CPR/BLS certification; board certification is preferred. A valid driver’s license and clean driving record are required. Competitive base salary in a comprehensive package.

Qualifications

• Licensed as a NP in the state where HarmonyCares operates.
• Federal DEA License/eligibility.
• Geriatric training/experience preferred.
• Active CPR/BLS Certification.
• Board Certification.
• Valid driver’s license with good driving record.

Responsibilities

• Provide in-home primary care to high-acuity patients in home health, geriatrics, hospice and primary care.
• Deliver patient-centered care with time to address each patient’s needs, not only volume.
