Abode Care Partners is seeking a compassionate Nurse Practitioner to join our Springfield, MO team. You will provide primary care to seniors and patients with complex needs in home visits, conducting assessments and creating personalized care plans.

Qualifications

• Licensed as a Nurse Practitioner with MSN and national certification.
• Active state NP license and DEA registration required.
• Proven ability to perform primary care assessments for seniors in home settings.
• Experience with EMR systems and clinical documentation.
• Strong communication and time-management skills; independent worker.

Responsibilities

• Perform primary care assessments and develop personalized care plans in home settings.
• Treat chronic and acute conditions, coordinate with interdisciplinary teams.
• Order and interpret labs and tests as needed, ensure up-to-date medical records.
• Provide referrals and support preventative health initiatives.
