Qualifications

• For NP positions, an MSN is required; PA degree acceptable with NP path.
• Active state license and national NP/PA certification.
• Current CPR and DEA licensure in state employed.
• Experience in primary care for seniors preferred.

Responsibilities

• Provide primary care services in home and community settings.
• Conduct clinical assessments and develop personalized care plans.
• Manage chronic conditions and prevent acute visits.
• Coordinate with interdisciplinary teams for seamless transitions.
• Document and update patient medical records accurately.
• Perform referrals to other healthcare practitioners when needed.
