Abode Care Partners is seeking a compassionate Nurse Practitioner to join our Springfield, MO area team providing personalized primary care in patients' homes. You will conduct assessments, develop care plans, and coordinate with interdisciplinary teams to improve outcomes.

Qualifications

• Active state NP license.
• Current NP National Board Certification.
• Active state DEA licensure.
• Master’s degree in Nursing (MSN) required.
• Five or more years of nursing experience; NP experience preferred.
• Strong interpersonal communication skills.
• Excellent time management with daily workflow accountability.
• Proficiency in EMR systems and Microsoft Office.
• Licensed driver with automobile insurance.
• Ability to engage in moderate physical activity and lift up to 25 lbs.

Responsibilities

• Provides primary care in home settings for seniors and patients with complex needs.
• Performs clinical assessments and develops personalized care plans.
• Collaborates with interdisciplinary teams to ensure seamless transitions.
• Manages chronic conditions and acute visits with preventive focus.
• Maintains up-to-date patient records and documentation.
