Job summary

Molina Healthcare seeks a Nurse Practitioner to provide screening, preventive primary care, and medical services in in-home, community, and nursing facility settings. You will coordinate care, perform exams, order labs, and create medical plans with physician oversight.

The role involves travel across the assigned region (up to 120 miles daily) and requires licensure, DEA prescriptive authority, BLS, and EMR proficiency. You will work collaboratively to improve outcomes and cost-effective care.

Qualifications

• 1 year+ experience as a nurse practitioner or equivalent.
• Active national NP certification (AANP/ANCC).
• Current valid FNP state license.
• DEA license with prescriptive authority.
• BLS certification.
• Valid driver's license and reliable transportation.

Responsibilities

• Provide medical care and care coordination for adult, pediatric, and geriatric populations.
• Perform history and physical exams to identify gaps in care and preventive needs.
• Order and interpret diagnostic labs and imaging as appropriate.
• Develop and implement a medical plan of care.
• Schedule visits and coordinate post-discharge care.
• Provide in-home, nursing facility, and community visits; travel up to 120 miles daily.
• Collaborate with physicians and participate in regional meetings.
• Prescribe medications and offer patient education.
