Job summary

Qualifications

• At least 1 year of Nurse Practitioner experience.
• Active national NP certification (AANP or ANCC).
• Current valid FNP license in state of practice.
• DEA license with authority to prescribe.
• Current BLS certification.
• Valid driver’s license and reliable transportation.

Responsibilities

• Provide general medical care and care coordination for adult to geriatric patients.
• Perform history, physical exams, and preventive assessments.
• Order and interpret tests; coordinate with specialists.
• Document medical plans in EMR and ensure timely medical records.
• Conduct in-home and community visits; travel up to 120 miles daily.
• Collaborate with physicians and participate in regional meetings.
• May prescribe medications and perform procedures as appropriate.
• Maintain cross-state licenses as needed.
