Job summary

Signify Health, part of CVS Health®, is seeking a Per Diem Physician to provide in-home health evaluations. The clinician will engage with individuals in their homes, review medical histories, perform physical evaluations, and coordinate with health plans for diagnostics when needed.

The role emphasizes flexible travel within licensed states, virtual visits when appropriate, and offering comprehensive services without prescribing or altering current treatments.

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Multi-state licenses (preferred).

• Provide all Signify Health services as indicated including Diagnostic & Preventive Services products (i.e., spirometry, DEE, bone density screening, etc.)
