Job summary

Signify Health is seeking a Part Time Physician to provide in-home health evaluations for members, visiting patients in their homes and coordinating with health plans. You will review medications, perform physical evaluations, and conduct required diagnostic tests as ordered by the health plan.

Role requires an active medical license (MD/DO) and willingness to travel locally within licensed states. Home visits, virtual visits, and comprehensive assessments are integral to improving patient care.

Qualifications

• Active, unrestricted license in required states.
• MD or DO degree from an accredited school.
• Experience with in-home or primary care assessments preferred.

Responsibilities

• Perform in-home health evaluations for members.
