Job summary

Signify Health, part of CVS Health, is seeking a Full Time Physician to provide in-home health evaluations, engaging with people in the comfort of their own homes and helping bridge gaps in care. You will use an iPad and an intuitive clinical workflow for each evaluation and are not required to prescribe medicine, order lab tests or alter people's current treatment regimen.

The position involves traveling locally and within licensed states, with virtual visits as needed.

Qualifications

• MD or DO physician with active, unrestricted license(s) in the applicable coverage area(s).
• Multi-state licenses (preferred).
• Active, unrestricted license(s) in coverage area(s) required.

Responsibilities

• Conduct in-home health evaluations, ~45 minutes per visit, reviewing medications and medical history.
• Visit patients locally and within licensed states, travel as needed for in-home visits.
• Perform diagnostic and preventive services as indicated and document findings in workflow.
• Conduct virtual visits when required and coordinate with the care team.
