Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Experience conducting in-home health evaluations is preferred.

Responsibilities

• Conduct in-home health evaluations lasting about 45 minutes.
• Review medication history and perform physical evaluation.
• Coordinate with health plan to provide needed diagnostics and referrals.
• Document visits using the Signify Health workflow.
