Qualifications

• MD/DO required with active license in coverage area(s)
• Multi-state licenses (preferred)

Responsibilities

• Conduct in-home physician visits with patient-centered care
• Perform medication and medical history review during home visits
• Provide diagnostic and preventive services as indicated by health plan
• Travel locally and within licensed states as needed
• Utilize iPad and clinical workflow to document evaluations
• Coordinate with health plan and PCPs to ensure continuity of care
