Job summary

Signify Health, part of CVS Health, is seeking a Full Time Physician to perform in-home health evaluations with patients in their homes nationwide. The visit lasts about 45 minutes and includes history review, physical assessment, and optional diagnostic tests as ordered by the health plan.

Responsibilities include building trusted relationships, visiting millions of people nationwide, and collaborating with clinicians. Licensed MD/DO physicians are required.

Qualifications

• MD or DO license in coverage area(s).
• Active, unrestricted medical license(s) in coverage area(s).
• Willingness to travel locally within licensed states.

Responsibilities

• Conduct in-home health evaluations for members using iPad workflow.
• Review medication history and perform physical evaluation during visits.
• Provide diagnostic and preventive services as indicated by health plan.
