Signify Health, part of CVS Health, is hiring a full‑time physician to provide in‑home health evaluations. You will visit members in their homes, review medications and medical history, and perform physical evaluations while coordinating with care teams.

Eligible candidates hold an MD or DO, with active licenses; multi‑state licenses are preferred. Expect local travel and occasional virtual visits as part of a national clinician network.

Responsibilities

• Visit health plan members in their homes to perform in‑home health evaluations.
• Engage with individuals to bridge gaps in care and provide comprehensive health assessments.
• Use a clinical workflow to conduct evaluations and coordinate with the health plan and care team.
