Signify Health, a CVS Health company, is seeking a part-time physician to perform in-home health evaluations. Visits last about 45 minutes with a focus on history, physicals, and preventive care, often traveling to patients' homes across licensed states.

You will use an iPad-based workflow to conduct evaluations, with optional virtual visits as needed and no prescription changes are required unless specified by the health plan.

Qualifications
