Signify Health, part of CVS Health®, is seeking a Full Time Physician to provide in-home health evaluations. You will engage with members in their homes, perform reviews, and deliver personalized health assessments using an iPad with a clinical workflow.

The role offers 40 hours per week, full-time status, and a comprehensive benefits package. Travel locally to licensed states is required as needed, with virtual visits available.

Qualifications

• Active, unrestricted medical license in the coverage area.

Responsibilities

• Conduct in-home health evaluations (~45 minutes) with medication and medical history review.
• Perform physical evaluation and provide health plan-driven services as requested.
• Travel locally across licensed states and conduct virtual visits when needed.
