Job summary

Signify Health, part of CVS Health, seeks a Full Time Physician to provide In-Home Health Evaluations, visiting patients in their own homes to bridge gaps in care.

Visits are about 45 minutes, with history, med review, and physical assessment. Travel locally within licensed states is required, using an iPad-based clinical workflow; prescribing is not required. Comprehensive benefits apply for full-time staff.

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Multi-state licenses (preferred).
• Able to cover Augusta, ME.

Responsibilities

• Bring compassionate care during home visits as part of a national clinician network.
• Perform in-home health evaluations including history, physicals, and medication reviews.
• Coordinate with health plans to connect patients with appropriate care.
• Travel locally and within licensed states as needed; conduct virtual visits when required.
• Use iPad and clinical workflow; no medicine prescribing or lab orders required.
