Job summary

Signify Health, part of CVS Health, is seeking a Full Time Physician to provide in-home health evaluations. Clinicians visit members in their homes, perform medical history reviews, physical evaluations, and coordinate with health plans to bridge gaps in care.

You'll work with a national network of clinicians, travel locally and within licensed states as needed, and use an iPad and streamlined workflow for each evaluation. The role offers a comprehensive benefits package and 40-hour work week.

Qualifications

• Active, unrestricted medical license in coverage area(s).
• Multi-state licenses preferred.
• Experience as a physician delivering in-home health evaluations is a plus.

Responsibilities

• Conduct in-home visits for health evaluations.
• Review medications and medical history during visits.
• Provide diagnostic and preventive services as indicated.
• Coordinate with health plans and primary care physicians.
