Job summary

Signify Health, part of CVS Health, is seeking a Full Time Physician to provide In-Home Health Evaluations, visiting members in the comfort of their homes and helping bridge gaps in care. You will perform 45‑minute visits, review medications and medical history, conduct a physical evaluation, and order additional diagnostic tests as guided by the health plan, using an iPad and clinical workflow.

You’ll join a national network of clinicians dedicated to improving health outcomes.

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Multi-state licenses preferred.
• MD or DO degree required.

Responsibilities

• Bring your heart into every visit, joining a national network of purpose-driven clinicians dedicated to improving health outcomes.
• Visit millions of people nationwide to connect them with the right care for their unique needs.
• Work with Signify Health as a clinician colleague, fulfilling the requirements of the specified role.
• Be flexible to travel locally and within licensed states, as needed.
• Conduct virtual visits as needed.
• Provide all Signify Health services as indicated including Diagnostic & Preventive Services products.
