Job summary

FOX Rehabilitation is expanding in Danbury, CT, and surrounding areas, seeking licensed Physical Therapist Assistants for in-home and senior living care. You will work closely with a supervising Physical Therapist to implement individualized treatment plans, guide patients through exercises, and help rebuild strength, mobility, and confidence in patients’ homes and senior living communities.

Qualifications

• Must hold a valid PTA license in CT.
• Experience working with older adults in home or community settings preferred.
• Demonstrates strong interpersonal and teamwork skills.

Responsibilities

• Deliver individualized treatment plans established by supervising PT.
• Instruct, motivate, and safeguard patients through therapeutic exercises and functional activities.
• Document visits accurately in the EMR per Clinical Documentation standards.
• Coordinate care with POAs and community referral sources and assist with coverage when needed.
