Clear Concepts, Inc. is seeking a dedicated Physical Therapist to join our home health team in the Denver area.

This full-time role focuses on delivering high-quality patient-centered care in patients’ homes, performing evaluations, and coordinating treatment plans with physicians and a multidisciplinary team. The ideal candidate will have at least nine months of PT experience, a CAPTE-approved master’s degree, and a valid state license plus CPR certification.

Qualifications

• Minimum nine months experience as a physical therapist.
• Preferred community/home health experience.
• Master’s degree in physical therapy from CAPTE-accredited program.
• Licensed to practice as a physical therapist in the operating state.
• CPR certification required.
• Licensed driver capable of travel to all locations.

Responsabilités

• Provides patient care and documents treatment per physician orders, agency policy and patient needs.
• Performs patient evaluations and selects diagnostic materials to assess patient care.
• Informs physician of assessment results and recommended interventions.
• Develops and documents treatment plans with appropriate interventions and priorities.
• Provides organized, efficient treatment including education to the patient.
• Presents patient information to physicians and keeps team informed of status and plan.
• Delivers treatment within agency and payer parameters.
• Sets objective, measurable goals in collaboration with team and patients.
• Meets productivity standards and uses work hours efficiently.
