Job summary

UnitedHealth Group's HomeChoice Health Services seeks a licensed Physical Therapist to evaluate patients, develop and implement care plans, and collaborate with the care team in community settings.

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.

Responsibilities

• Evaluates the patient's functional status and physical therapy needs, and assists in developing the plan of care.
• Provides services within the scope of practice per state laws and plan of care.
• Observes and reports patient response to treatment to the supervising nurse or physician.
• Treats patients to relieve pain, restore function, and maintain maximum functioning.
• Instructs patient and family regarding the plan of care and progress toward goals.
• Visits patients per Plan of Treatment and submits progress notes.
• Completes evaluations and develops the PT plan of care within state guidelines.
• Reports outcomes of evaluation, goals, and anticipated frequency of care.
• Function in any home situation regardless of demographic factors.
