Job summary

UnitedHealth Group is seeking a Physical Therapist in Home Health to assess patient needs, develop plans of care, and deliver interventions aimed at improving mobility and function in home settings. The role requires current physical therapy licensure, a valid driver's license and reliable transportation.

Candidates should appreciate teamwork within a multidisciplinary health care team and be committed to delivering compassionate, high-quality care.

Qualifications

Current Physical Therapy licensure in state of practice.
Current driver''s license and vehicle insurance, access to a dependable vehicle, or public transportation.
Preferred: Current CPR certification or ability to complete within 90 days of hire.

Responsibilities

Evaluates the patient's functional status and needs; assists physician in plan of care.
Provides PT services within scope of practice and plan of care.
Observes and reports patient response to treatment and changes in condition.
Treats patients to relieve pain, restore function, and maintain maximum functioning.
Instructs patient and family about plan of care, limitations and progress.
Visits patients per Plan of Treatment and documents progress notes.
Completes all evaluations and develops PT plan within state guidelines.
Reports outcomes and anticipated frequency of care.
Function in diverse home environments.
