Qualifications

• Current Physical Therapy licensure in state of practice.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
• Current CPR certification or ability to complete within 90 days of hire.

Responsibilities

• Evaluates the patient's functional status and physical therapy needs. Assists the physician in the development of the therapy plan of care.
• Provides services within the scope of practice as defined by state laws and in coordination with the health care team
• Observes and reports patient response to treatment and changes in condition to the supervising nurse/physician
• Treats patients to relieve pain, develop and restore function, and maintain maximum functioning
• Instructs patient, family, and caregivers regarding plan of care and progress toward goals
• Visits patients per Plan of Treatment and submits progress notes per policy
• Completes patient evaluations and develops the PT plan of care within state guidelines
• Reports outcomes of evaluation, goals, and anticipated frequency of care
• Function in diverse home settings regardless of age, race, creed, or disability
