Job summary

UnitedHealth Group invites experienced Physical Therapists to join a Home Health team providing patient assessments, care planning, and therapeutic interventions. You will evaluate functional status, mobility and pain, develop treatment plans, and monitor progress in a home-care setting.

Responsibilities include collaboration with physicians and nursing staff, documenting progress, and ensuring adherence to state practice guidelines.

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current driver’s license and vehicle insurance or access to dependable vehicle.

Responsibilities

• Evaluates the patient’s functional status and needs and assists in developing the plan of care.
• Provides PT services within scope of practice and plan of care.
• Observes and reports patient responses and progress to supervising staff.
• Implements interventions to improve and restore function and reduce pain.
• Informs patient and family about plan of care and goals.
• Completes evaluations and documents care progress per policy.
• Ensures plan of care complies with state guidelines.
