Job summary

Hireology is seeking a licensed Physical Therapist in California to provide professional PT services within standard practice, collaborate with primary care physicians and health care team, and educate patients and their families.

The role includes intake, history, PT assessment, home safety evaluations, developing and submitting the Plan of Care, reassessment, and coordination with physicians to support home health therapy.

Qualifications

• Completion of a CAPTE-approved PT program and licensure in CA.
• Current CPR certification.
• Ability to communicate effectively in English.

Responsibilities

• Obtain medical history and intake information for visits.
• Perform PT assessment and document findings.
• Develop and submit PT Plan of Care (POC) for physician approval.
• Conduct home safety and environmental assessments.
• Coordinate care with physicians and other health team members.
• Perform ongoing reassessments and document progress.
