Job summary

Beyond Home Health Care is seeking a licensed Physical Therapist to provide in-home and community-based PT services across New Jersey for adults with developmental disabilities. You will evaluate mobility, strength, balance, gait, and functional movement, and deliver person-centered interventions per NJ DDD guidelines.

The role offers per diem/contract hours with a flexible schedule, mileage considerations, and supportive leadership for caseload management.

Qualifications

• Current New Jersey license as a Physical Therapist (PT) issued by the NJ State Board.
• Graduate of a CAPTE-accredited physical therapy program (DPT or equivalent).
• Minimum of one year of clinical experience preferred; new graduates considered.
• Current CPR/BLS certification.
• Clean criminal background check and fingerprinting per NJ DDD requirements.
• Reliable transportation, valid NJ driver's license, and current auto insurance.
• Ability to pass DDD-required background checks including CRoff.

Responsibilities

• Conduct comprehensive evaluations and develop individualized treatment plans aligned with ISP and goals.
• Deliver one-on-one therapy services in home and community settings.
• Document services per DDD standards with notes and outcomes, submitted on time.
• Collaborate with Support Coordinators, families, caregivers, and the team for continuity of care.
• Provide caregiver training and home programs to reinforce goals.
• Adhere to DDD policies, HIPAA, and required trainings.
• Maintain accurate billing records with DDD rates and authorizations.
