Job summary

Metro Physical & Aquatic Therapy is seeking a motivated per diem Physical Therapist for in-home care in Suffolk County, including Melville/Dix Hills. The position welcomes new graduates with mentorship and offers a flexible schedule to accommodate patient needs.

Licensed NY PT or eligibility is required; home health experience is preferred. You will evaluate patients in their homes, develop individualized plans, and document visits while collaborating with the team to ensure high-quality patient

Qualifications

• Graduate of Physical Therapist program.
• Licensed Physical Therapist in the state of New York or eligibility.
• Home health experience preferred but not required; mentorship available for documentation/compliance.
• Ability to travel to patients’ homes within the assigned geographic territory.

Responsibilities

• Accept and treat patients within your designated geographic area.
• Evaluate patients’ needs for physical therapy in the home setting and develop individualized treatment plans.
• Develop a plan of care and treatment goals in consultation with the physician, patient, and family.
• Plan, prepare and implement customized therapy programs to restore function and promote independence.
• Instruct family members and caregivers on appropriate therapy techniques.
• Provide timely and accurate documentation for each visit.
• Communicate effectively with patients, families, and interdisciplinary team members, especially around discharge planning.
• Deliver care in compliance with agency policies and professional practice standards.
