Metro Physical & Aquatic Therapy is seeking a motivated and passionate per diem Physical Therapist to provide in-home care across South Shore Nassau County. Flexible schedule and a mentorship program are available to support new graduates.

All levels of experience are encouraged to apply; the role offers 1099 and W-2 options and opportunities for professional growth within a supportive, innovative team.

Qualifications

• Licensed Physical Therapist in the state of New York or eligible.
• Physical Therapist Program Graduate.
• Ability to travel to patients' homes within the assigned geographic territory.
• Home health experience is preferred but not required; mentorship provided for documentation and compliance.

Responsibilities

• Accept and treat patients within your designated geographic area.
• Evaluate patients' needs for physical therapy in the home setting and develop individualized treatment plans.
• Develop a plan of care and treatment goals in consultation with the physician, patient, and family.
• Plan, prepare and implement customized therapy programs to restore function and promote independence.
• Instruct family members, PTAs, Home Health Aides, and caregivers on appropriate therapy techniques.
• Provide timely and accurate documentation for each visit.
• Communicate effectively with patients, families, and interdisciplinary team members, especially around discharge planning.
• Deliver care in compliance with agency policies and professional practice standards.
