Job summary

Metro Physical & Aquatic Therapy is seeking a per diem Home Care Physical Therapist to provide in-home services across Western Suffolk County, including Melville. Flexible scheduling with 1099 and W-2 options, allowing travel to patients' homes and autonomous patient care.

Licensed NY PT or eligible required; all levels welcome and supported with mentorship. Essential focus on patient care and documentation in a progressive, supportive team environment.

Qualifications

• Licensed Physical Therapist in NY or eligible.
• Physical Therapist program graduate.
• Home health experience preferred but not required.
• Ability to travel to patients’ homes within the assigned geographic territory.

Responsibilities

• Accept and treat patients within your designated geographic area.
• Evaluate patients’ needs for physical therapy in the home setting and develop individualized treatment plans.
• Develop a plan of care and treatment goals in consultation with the physician, patient, and family.
• Plan, prepare and implement customized therapy programs to restore function and promote independence.
• Instruct family members, PTAs, Home Health Aides, and caregivers on appropriate therapy techniques.
• Provide timely and accurate documentation for each visit.
• Communicate effectively with patients, families, and interdisciplinary team members, especially around discharge planning.
• Deliver care in compliance with agency policies and professional practice standards.
