Metro Physical & Aquatic Therapy is seeking a motivated and passionate per diem Physical Therapist to provide in-home care. All levels of experience are encouraged to apply and will train the right new graduate. Pay is $70-$90 per visit; location includes Suffolk County, Melville/Dix Hills.

Qualifications

• Licensed Physical Therapist in New York or eligible.
• Graduate of a Physical Therapist program.
• Willing to travel to patients' homes within the assigned geographic territory.

Responsibilities

• Accept and treat patients within your designated geographic area.
• Evaluate patients’ needs for physical therapy in the home setting and develop individualized treatment plans.
• Plan, prepare and implement customized therapy programs to restore function and promote independence.
• Instruct family members, PTAs, Home Health Aides, and caregivers on appropriate therapy techniques.
• Provide timely and accurate documentation for each visit.
• Communicate effectively with patients, families, and interdisciplinary team members, especially around discharge planning.
• Deliver care in compliance with agency policies and professional practice standards.
