Metro Physical & Aquatic Therapy is seeking a motivated and passionate per diem Physical Therapist to provide in-home care in Western Nassau County near Westbury. All levels of experience are encouraged to apply and will train the right new graduate.

Job type per diem with pay per visit in the range of $70–$90. We offer 1099 and W-2 options, competitive salary, growth opportunities, flexible schedule, and a supportive team.

Qualifications

• Licensed Physical Therapist in NY or eligible.
• Home health experience is preferred but not required.
• Ability to travel to patients' homes within the assigned geographic territory.

Responsibilities

• Accept and treat patients within your designated geographic area.
• Evaluate patients' needs for physical therapy in the home setting and develop individualized treatment plans.
• Develop a plan of care and treatment goals in consultation with the physician, patient, and family.
• Plan, prepare and implement customized therapy programs to restore function and promote independence.
• Instruct family members, PTAs, Home Health Aides, and caregivers on appropriate therapy techniques.
• Provide timely and accurate documentation for each visit.
• Communicate effectively with patients, families, and interdisciplinary team members, especially around discharge planning.
• Deliver care in compliance with agency policies and professional practice standards.
