Metro Physical & Aquatic Therapy is seeking a motivated per diem Physical Therapist to provide in-home care in Suffolk County, NY, including Melville and Dix Hills. All levels of experience are encouraged to apply and will be trained as needed.

Flexible schedule and supportive team environment are emphasized. Responsibilities include evaluating patients in the home setting, developing individualized plans of care with physicians, and delivering therapy to restore function.

Qualifications

• Licensed Physical Therapist in NY or eligible.
• Home health experience preferred but not required—mentoring offered.

Responsibilities

• Accept and treat patients in their homes within the geographic area.
• Evaluate PT needs and develop individualized in-home treatment plans.
• Plan and implement therapy programs to restore function and independence.
• Instruct family members and caregivers on therapy techniques.
• Provide timely, accurate documentation for each visit.
• Communicate with patients and families, and discharge planning with the care team.
