Job summary

Ignite Rehab (DBA Metro Physical & Aquatic Therapy) is seeking a motivated part-time Physical Therapist to provide in-home care across Essex County. This role offers 20 visits per week with benefits and flexible scheduling.

All levels of experience are encouraged to apply, and a mentor is provided. Licensing in NJ or eligibility is required, along with travel to patients' homes within the assigned geographic territory.

Qualifications

• Physical Therapist Program Graduate
• Licensed Physical Therapist in New Jersey or eligible
• Preferred Certifications: First Aid, AED, & CPR Certifications.

Responsibilities

• Accept and treat patients within your designated geographic area
• Provide 1:1 treatment
• Evaluate patients and develop individualized treatment plans based on physician orders and patient needs
• Deliver care that aligns with professional standards, agency policies, and the prescribed plan of care
• Educate family members, home health aides, and other caregivers on proper therapy techniques
• Plan, and carry out occupational therapy programs to maintain, improve or restore physical function, relieve pain, and prevent occupational dysfunction in patients.
• Perform and document an initial exam, evaluating the data to identify problems and determine a diagnosis prior to intervention.
• Provide timely and accurate documentation for each visit
• Evaluate effects of treatment at various stages and adjust treatments to achieve maximum benefit
• Must be willing to treat geriatric patients.
