Job summary

Ignite Rehab, based in New Jersey, is seeking a motivated part-time Physical Therapist to provide in-home care. The ideal candidate will have a strong clinical background and will be responsible for evaluating patients and delivering individualized treatment plans.

Job Description

We are looking for a motivated and passionate part‑time Physical Therapist with benefits to provide in‑home care. All levels of experience are encouraged; we will train the right new graduate. This is a part‑time position with 20 visits per week, located in Union County, with a flexible schedule.

Responsibilities

• Accept and treat patients within your designated geographic area.
• Provide 1:1 treatment.
• Evaluate patients and develop individualized treatment plans based on physician orders and patient needs.
• Deliver care that aligns with professional standards, agency policies, and prescribed plan of care.
• Educate family members, home health aides, and other caregivers on proper therapy techniques.
• Plan and carry out occupational therapy programs to maintain, improve or restore physical function, relieve pain, and prevent occupational dysfunction in patients.
• Perform and document an initial exam, evaluating the data to identify problems and determine a diagnosis prior to intervention.
• Provide timely and accurate documentation for each visit.
• Evaluate effects of treatment at various stages and adjust treatments to achieve maximum benefit.
• Must be willing to treat geriatric patients.

Qualifications

• Graduate of a Physical Therapist Program.
• Licensed Physical Therapist in New Jersey or eligible to obtain license.
• Preferred certifications: First Aid, AED, CPR.
• Home‑health experience preferred but not required; mentorship provided for documentation and compliance.
• Ability to travel to patients’ homes within the assigned geographic territory.

Additional Information

The anticipated base salary range for this position is $65,000–$75,000. Salary is based on various factors, including relevant experience, knowledge, skills, other qualifications, and geography. Medical, dental, vision, 401(k), paid time off, and other benefits are also available, subject to the terms of the company’s plan.
